wait, i was looking at their info, and it looks really annoying and difficult to deal with them, and just wow, look at their info, you have to jump though a lot of hoops to get them to take your work off their site, and then claim its not their fault it was on their in the first place. so i am extremely sorry to all that they stole the works to because it looks like it will be anything but easy to get them to take it down.
Some templates have been offered for the form, but I notice that they warn you off sending a message to their ISP so that it won’t slow down their response due to the complaint being filed wrong. Yeah, that and they don’t want you telling on them…

I have a very clear statement that says I do not want my works or translations published on any platform other than AO3, for various reasons, yet this seems to have been completely ignored.
